After testing I thought your work was very well implemented, and you did a really nice job. I'm not fully certain I'll keep this installed, though. Only because I don't know if it fits into my head canon. What I mean is that, for me at least, Witcher 3 is a compellingly gritty world, and the music conveys that. Skyrim is a very different kind of beast. But, that's my problem, not yours.
One thing I'd suggest is that you put the Requirement for Personalised Music bolded, at the top, and also add a requirement popup when people download this. In addition, the link you provide for Personalised Music in your readme actually goes to the Classic Skyrim version, you may want to update that. In my experience on these forums...people rarely read anything, so I thought I'd make mention of those two things.
