Modder kiki put together a plugin for ECE ygNord Race that marries a custom "female" head mesh with a male body. You can make these types of characters easily with that.
Here is what I have. A couple of things: 1) I won't provide ANY support. Either you know how this works or you don't. 2) It's LE format, which is what I use for character creation. I simply corrected defective shader settings in the meshes.
I know the feeling, haha, I still haven't figured it out myself. Albert is a standalone follower made by oipami. I believe they manage it by somehow swapping male and female head meshes around, but I need to just set some time aside and figure out how exactly modders like oipami are managing to create such awesome characters. You may want to look into grabbing ECE (Enhanced Character Edit) and the resources from El's Severini follower to get started. Using those two together after swapping out meshes and textures has allowed me to create some decent player characters, but still nothing as great as Albert and other such followers.
Essentially you would manually go into your textures and meshes folders under Data and replace either your existing textures and meshes under Character Assets (which would affect most all base game NPCs) or into your custom race's individual folders, like with ECE's ygNord race.
