Technically yes. AS will send to a random "start" spot that the author made, but I am assuming you can just revisit Helgen when it's in ruins and be able to find it
I have not used Alternative Start myself so I can not answer your query. I have been to Helgen many times when it was destroyed and you still have access to Helgen Keep. There are two other external doors giving you access to th Warehouse Crafting area. One in Whiterun and one in Falkreath.
