If you are a vampire at stage 4, you will be attacked on sight like it was before the Dawnguard DLC. No .Esps, only a script change.
Note: If you are a vampire at stage 4 in your current game (midgame), you will have to feed to reset your stage to level 1 and progress again to stage 4 for this to work.
