"unknown" means that the quest giver doesn't know what's at the target location, and therefore a lot of different location types can be selected. I don't think Troll locations have their own keyword, the existence of Falmer is not know enough by the regular people aboveground to specifically send you there (although falmer dungeons can be selected as part of the "unknown" bounties, and I don't see a reason why a Jarl would place a bounty on a draugr...
She gets killed and then necro-ed when you first enter that chamber, depending on what quest mods are touching that scene. Sometimes she is already an ashpile before you enter the cave, it seems. Something to do with the timing of it all. Anyway, that would be cool if a scene with something similar happening to her in a necro habitat still occurs.
Sounds pretty cool, but quest items just lying on the ground are begging to be perma-lost. Loving the randomness of some of the quests instead of "this item is always in this place."
