I figured it out now. I move the camera forward with the "auto run toggle"-button (default caps lock, which I've rebound too) M1 moves the camera up along the z-axis. This and M2 moving it down works pretty ok, with some getting used to.
With the default key-mappings, what do you use to move the camera up and down?
I've played around with the free cam mode, but can't seem to be able to move forward with it. I've bound M2 for foward and pressing that whilst in free cam mode the camera moves down :/ Strafe and reverse works, but its tedious to reverse into position to get a good screen shot.
So, which button to press to move the camera forward? (I guess I have to rebind my buttons, but I'm hoping there's some hidden default button to use)
