some more notes: - use the potion of jump toad to increase acrobatics to jump up to where the suit of armour stands above the entrance to the crypt (as alluded to by the poem you find on some dudes dead body). There is a lever behind the suit that opens a small room with a chest with the key to the butchers room inside - to get the crystal near maras shrine that's just out of reach, use the potion of folly as suggested by the text on the tombstone set in the wall near maras shrine, then walk through the tombstone. A secret hallway will open leading to the chamber where the crystal is - for me, once I reassembled Ivan, he would only continue to follow me in the crypt level. If I go up or down to other levels, he would stay behind, so sad

there's just one more thing missing in your walkthrough. I think no one ever found it, except people who took a closer look at the book textures. it's a small easter egg in the dungeons (level 2): there's a grid in the floor next to the well. pass it via tcl and look for the skeleton on the ground.
also, as for the lever behind the armor you need to find to get to the butcher: brutus has a poem which refers to it. i guess it's not that obvious. there sould be a potion in the near which increases acrobatics, so it shouldn't be a problem to reach the lever.
when you're on the stairs that lead to the ower level of the crypt, don't go down, but go across the balcony to the other side. Don't take the left, or the right, and you'll end up in a small room with a skeleton in it. That's him.
