If you don't notice a difference then you either installed it wrong, or you need listen harder. Just listen to the vibration in the loud parts, and the mixed in combat music in the quiet parts.
To install, just download and extract the music file. Then place it in C:\Program Files\Bethesda Softworks\Oblivion\Data\Music\Special (you might want to move the old one first in case you still want it.)
