After decades of nightmares and ears bleeding, here's a fun and lore friendly solution to silence Mick and Ralph's screamer.
When you talk to him, three solutions to solve your noise problem will be offered to you:
Note: he will stop to bug you when you're passing by but will keep on if it's another NPC. Also add other conditions so he does not say anything when the rat from Freeside, your followers and other kids are passing by.

- Threaten him (bad for your reputation and your karma, come on, he's just a kid!)
- Buy his silence for 250 bottle caps (no effect other than to lighten your pockets).
- When using a mod restoring Child at Heart perk, you can use it to silence him for free.
