An ENB preset. Download ENBSeries 0.451 for Fallout New Vegas, then move the .ini files from the download to your Fallout New Vegas directory, the folder that contains the Data folder and the game .exe. Detailed shadows are disabled since they cause transparency bugs for me, but if you don't have that problem, then just enable them.
I lost just 2 fps in Camp McCarran's terminal building, from 15 fps down to 13 fps. Not saying you should install an ENB when you're getting 15fps, but it runs pretty well most of the time, only limited by the game's optimization only letting my GPU go up to around 27% usage randomly.
If you're really hurting for performance, disable ambient occlusion, detailed shadows, and lens effects.
