It takes water to sterilize the tools AND it takes water to run the operation, so it costs 3 water every use. I think it would make sense if the Doctor's Bag's pathetically low amount of limb healing was increased if it's going to be so expensive to use it. The mod seems fun, but not well balanced.
How I recommend changing it: take water and pressure-cooker requirement out of sterilization; at a campfire you should be able to sterilize them in the fire itself, or it can be done with alcohol during the procedure. You might take out the sterilization process entirely just because it's redundant. When using the doctor's bag, it should require 1 purified water and 1 alcohol -- whiskey, vodka, scotch, or wasteland tequila if it's possible to enable multiple alcohol choices. If you have to pick one, pick scotch because the other three have other uses but scotch is a bit useless in vanilla.
The in-game doctor's bag isn't meant to be the bag of a doctor making house calls to people with the flu, it's more of a combat-oriented first aid kit.
You might want to make a version that allows you to do this in combat. Really, it's not realistic at all to say you can't perform life-saving medicine while in combat - doctors have done this for ages. You simply risk getting shot while in the middle of a procedure. Maybe make it take longer if in combat, and possibly have a chance of failing to correct the problem. That would be realistic.
Yup...thinking in FWE terms, that mod has a full-on "Triage" mode that allows you to fix limbs out of combat, using a Medical Brace and Surgical Tubing (which one it uses depends on what limb is crippled...Tubing for head and chest, a Brace for arms and legs), but when you're being shot at, you can only apply "First Aid", which uncripples the body part in question, but returns it to only one hit away from being crippled...always thought that was fair.
I'm not saying your mod is wrong, but when you are crafting doctor's bags, you need forceps and scalpels to make them, so they would already have those two items in them. So maybe you could make them need duct tape (as a placeholder until you decide to make surgical tape), or require a medical brace to work. You don't need to do this, I was just giving you an idea
This is essentially how the mod works as it stands. I took scalpels and forceps out of the doctor's bag (you still need them in your inventory) and replaced them with scissors, tweezers, duct tape and wonderglue. Part of the idea behind that substitution, aside from any notions I might have about credible post-apocalyptic medical kit, is that it both makes use of what are otherwise junk items and puts a drain on certain resources perhaps you'd prefer to spend on, say, weapon repair kits or explosives.

I feel as though this is difficulty for the sake of difficulty and not for realism or immersion.
