This is just a saved game with a decent looking face. I saved the game as "newguy" right as I was getting off of Doc Mitchel's bed at the very beginning of the game.
The only thing that has been done in game was to give the newguy a name. His name is Pistol Pete, but can be changed when you exit Doc Mitchel's, or when you leave goodsprings.

Install:
Unzip the newguy.zip to anywhere.
copy newguy.fos to My Documents/My Games/FalloutNV/Saves
Start FNV and load the game, "newguy"
Ignore the warnings about this save relying on other files.

Alternatively You an get Face ripper from below, rip the
face code from newguy.fov and add it to your current
saved game.
