    That looks like a weapon mesh/animation problem to me. I've had similar weirdness happen when I've tried to re-rig a weapon to work with a new animation and gotten it wrong.

    Do the mis-aligned parts (like the SMG magazine) still move during animations (eg. reload)? Any animation mods installed? Any mods that alter the models?

      The weird thing is it eventually fixes itself but reloading a save does not

      Can you pin point any specifc action that triggers/fixes the issue? Like bringing up the pip-boy, or switching to another weapon?

      I figured it out it was hitman47101s 12.7mm anim causing it when switching from it.

    yeah, animations used with wrongly named nodes.
    node names in the anim dont correspond to the node names in the weapon mesh.
    so the game doesn't know where to place the parts of the gun.

    use animations with proper node names.
