    His name is Opal Holtzer, better known as Holtz. He is one of the Failed FEV Subjects that suposedly died during Vault 87 experiments. In spite of the excruciating pain during the session, managed to survive, but was kept locked for a long time.

    At first his hideous appearance would startle the lone wanderer, but through conversation it would be clear that he meant no harm, and only wants to get out of the cell. Contrary to the original Failed FEV Subject, its body shape is closer to a normal human. The Vault dweller would provide him with a weapon and enlist him on his journey out of Vault 87.

    The effects the virus had on his body are pretty plain, and keeps suffering from it, so it would be the Vault Dweller mission to get him some kind of treatment to ease the pain, gaining him as a permanent companion in return.

    I LOVE the idea of this guy as a companion. The dialogue alone would be great.

    LW: "...."
    "Why havent you shot at me yet?"
    "What?"
    "Look I know what I look like...and also know that you didnt get this far by being a pacifist...so my question is, how do you know I wont just jump up right now and lunge for your throat?"
    "Because I'd put you in the ground otherwise?"
    "Hmm..true. Names Opal...everyone calls me Holtz. Can I help you with something?"

    Even just him commenting on places like underworld or the strip would be amazing.
    "Wow....I never thought I'd still be the ugliest guy in the room...thanks man..." (Underworld)
    "Hey, not to ruin the swanky atmosphere of this place, but I am fairly certain i just lost like a bit of muscle back there and some freak picked it up and walked to the kitchen with it." (Ultra lux).

    Like I could so see this guy as a Raul - esque character. Snarky and ready to call you out.

      Thank you Jackal!, I'm happy it was to your liking . I admire how natural, fresh and fitting are the dialogues you come up with for this characters. They would work nicely with Holtz and the player

    With that model (or something similar) you could also make Talius, a Former Vault 13 dweller FEV Mutant working with the Followers of the Apocalypse in Fallout 1.

      Thanks for the idea Ru5tyShackleford! . I was thinking about giving him a make over so he can stand out of the original Failed FEV Subject. Making a variation for Talius sounds like fun .
