Due to a known bug, if you start the "Of Life and Death" quest by going into a house instead of talking to the quest giver it may not count towards the achievement upon completion.
The other way you can miss this achievement is not returning the ring to Maisie Calhoun during the "Lucky Charm" quest
Just place the savefile into <Steam-folder>\userdata\<user-id>\1041720\remote\
Load the game. It should be called "Crownhold Garrison"
After the game loads simply walk outside and listen to the concert of of the Colet and the Echoes.
