This is called combat overhaul because it is an overhaul, but its pretty simple. You must match the attack direction  side in order to perfect block. Master Strikes must still be direction matched. Stabs can be MS and PB from any direction. The top zone can parry top and top left and top right

Here is a really shitty depiction of the PB sides. (Green is for the top zone only. Left and right can parry top as well)



The reason for sides is because
1. Direction matching is not realistic. Ripostes don't give you enough time to change angles, meaning you almost always got hit by them
2. You were straight dead in 1vX
3. The AI doesn't always match 100% so you can rush them
4. Combos would immediately ruin your day


It's not that this is necessarily realistic, because in real life you can ward an attack from any guard (and it can be better to have the opposite guard) but the perfect block is a little boring in this game, and you literally can't get around it by feinting. Feints don't really screw up PB in this game because the timing for PB is after the strike is released anyway. You can't morph your strike like in mordhau or anything.

Basically this makes defense harder and feinting better. Dodge is untouched though so you will need it for being outnumbered
