If you have discord it is best you join the Modding Discord and report your bug/issue there, as you'll get the fastest and best support.
If the game crashes : include the relevant part of the crash report (see images below) and describe in as much detail as possible how it happened. What were you doing? Were you at the house or in a run? What did you have equipped, etc.
If the game does not crash : please provide screenshots if the bug is visible, and describe in detail how it happened.
Whether the game crashes or not : please include your save file (more info below) in your bug report. It will make it far easier for me to replicate bugs and find a way to fix them.

Common errors :
When I try to open the Codex a red cross pops up and nothing happens? You need to have unlocked the Codex in order to open it. On a fresh save Achilles will give it to you after a few runs when you talk to him.
When I load my game it tells me save data is corrupted! Your save is just fine, however the mod was not installed correctly (probably missing Mod Utility). When the game cannot load your save for any reason it will show this error message. I have a mod that replaces original game files and the game crashes when I try to use Codex Menu! Those mods are called old format mods and are not usable with Codex Menu. Do an integrity check in your launcher (Steam/Epic/Gamepass) or restore your files, then launch modimporter.
I'm missing the boon selector in the codex even on a brand new save and getting the codex, only see boon manager and up options.
EDIT: Dug deeper into this and looks like it still works but probably changed due to PonyQOL mod's interaction.

For the hammers, just go to hammer's entry under the Artifacts tab and hit Enter.
As I've replied on that bug report (which isn't a bug), this is how codex menu has always worked, and is explained in the description. It's purely user error and if you don't read the mod description, which explains how each feature works and how to use it, that's your fault.
I'm not even getting a Normal Boon Selector. However, the Duo Boon Selector does appear, though it only shows a blank screen. I did discover that the boons were just hidden behind the selector, and you can click on them even if you can't see them. I assume this would be the case for you and the Normal Boon Selector, but I wouldn't know because I can't even see it lol.
