Trust me, downgrade GTA IV to 1.0.4.0 and then try my last Remaster pack, you won't want to go back to normal GTA IV. The name of the pack is GTA IV DEFINITIVE EDITION MODPACK BY STEFANOTTO_88, it's quite heavy so if you're interested i recommend you to watch the viddo i uploaded first. At the moment i'm working on a new version even better, hope you'll try it! :D
