This is a simple model swap of very common bloated glowing ones to look more withered, like the ghouls encountered during Radiation Rumble Event, inspired by withered charred ghoul replacer mod by CatsWithMachineGuns.

This mod changes the model of bloated glowing ones that appear around in the world and in nuke zones and swaps it to one of the 3 different withered glowing ghoul models. Like CatsWithMachineGuns said I also find the ghouls scarier if they look withered and the bloated ones I just find really repulsive and not that much creepy. As soon as I had the right tools to correctly unpack the Fallout 76 ghoul meshes and correctly pack them into a F76 compatible .ba2 file it was just a 5 minute work.
There are 3 variants to choose from, but only one replacer can be active during gameplay, you can see on pictures how each version looks because there is a model for a normal glowing one and 2 different models for withered putrid glowing ones.
