This mod was made so you can "wear" a certain set of perks under the form of wear-able "Implants" aka items that take no item slot, or gear that was previously lacking any stats - like bandanas/surgical mask, so you can have those annoying utility and crafting perks without spending precious points into them.
The items and their associated perks are split somewhat logically, so you can choose what you want to wear.
The items give you the perks at max level as long as they are equipped, and revert to whatever valued you had them before when you un-equip them.
You can find all the items inside a special safe within HOME PLATE player house within Diamond city.
