Craftable in white and black at chemistry workbenches under the "Respirators" category. There are also separate hat compatibility versions marked as (HC), which are sort of unofficial and unsupported because they clip with basically every piece of headgear in the game.
It has the same stats and functionality as any default gas mask in the game.
