Working great for me with Survival Options and Survival Stats Widget Redone, along with hundreds of other mods. Just throw it at the bottom of your load order.
Not working 2021...unfortunately I was wrong! and glad I was. It is working - so far ive tested for hydration - it just takes quite a long time to change colors. To test it, use this with another mod that enables console in survival mode, then spawn 7 stimpaks. Consume 3 of these and watch the hydration icon change color to yellow, then red. Really like this mod. Thank you x100!
How does this mod work exactly? Are the icons always showing on the HUD, and where on the HUD? Does it only appear when you have less than %100 survival needs? Do I have to turn it on in the MCM?

Survival and HUD mods may over write this mod making it not work. This is one of the easiest mods to fix with FO4Edit. Load order changes may fix it also. Don't fear, FO4Edit is your friend.
If you have no survival icons change in game settings - display - effects icons. If that does not work try replacing fallout4pref.ini. There are other fixes you can find on the internet.
