You can modify your facial features in the comfort of your own Megaton House or Tenpenny Suite! Just tell your Robobutler (Godfrey / Wadsworth) that you want a plastic surgery and you can modify your facial features free of charge.
Previously "plastic surgery" could only be done by Pinkerton in Rivet City after "The Replicated Man" quest.
