    ** SPOILER WARNING **

This section is for those having issues completing the quest / knowing what they're supposed to do next. Please note that this does not encompass every possible way of completing an objective - the mod was designed so that you can learn information required to progress through the story in many different ways (dialogue, specialty dialogue, terminals, evidence found, cross-examination, etc). All the same, if you're stuck: this should get you through.

How do I start the quest?
Travel to the Chryslus Building. In the parking lot outside, you'll find a corpse in an unusual radiation suit. Loot the body, making sure to take the reassignment papers, the radiation suit and the keycard. This will start the quest and give you the objective of traveling to the IRC.

    Where is the IRC?
The entrance is located at Vault 87 - a vanilla area that was until now unreachable due to the high levels of radiation. Normally, the door doesn't link to anywhere since the player isn't able to reach it, but now it leads to a cave.

I keep dying when I try to make it to the entrance?
As the reassignment note warns, you won't survive the radiation levels without the special IRC radiation suit that you got off agent Mathers' body. Even with the suit, don't dawdle - the radiation levels are still very high to ensure the player can't survive without the suit.

I die whenever I go back outside?
Make sure you're wearing your Institute radiation suit and check your rads: if they're already high, you may not survive the trip back even with the special suit. Terry in the IRC can cure your radiation sickness for free. If all else fails, use tgm in the console to survive the trip.

I found the caved-in vault door but not the IRC?
Retrace your steps: there's a somewhat hidden side patch that leads to a hole that's been blown into the vault. Here you'll find the IRC.

I can't get inside - nothing happens when I walk through the lasers?
You need to be carrying the keycard that's looted from agent Mathers' corpse, outside the Chryslus Building.

I'm "exploring the IRC" - now what?
Explore! Talk to people, read the info terminal like Penny says, check people's email - there are a number of things that will get you to the point of realizing something strange is going on here.

I can't progress past "the staff are hiding something - find out what"?
Check Terry's email: one of the ones from Galen will clue you in to the fact that Galen and Terry were friends. As long as you've established from talking to the staff WHO Galen is, you can confront Terry about his disappearance.

    Where is Galen's insurance policy?
It's a holotape under one of the beds in the staff quarters.

How do I decode the holotape?
Talk to Terry and tell him you've found the insurance policy. He'll give you the cipher and you can now use his computer to decode it and obtain the password to Rutherford's computer. Interesting side note: the array of numbers in the holotape isn't nonsense. Follow the instructions in the cipher and you'll get the same decoded message.

I opened Ivanov lab from Rutherford's computer but where is the lab entrance?
In the short hallway that connects the medical wing with the research wing, you'll find a secret door now open.

I die whenever I walk through the beams in Ivanov lab?
After decoding the cipher and/or checking Rutherford's computer, make sure you talk to Terry. He'll explain that in order to access the lab, you're going to need Rutherford's keycard. If your sneak skill is high enough, you can steal it from him, but otherwise you'll have to talk to Penny, who (depending on your skills) you can seduce, blackmail or bribe into helping you.

I found Galen - what the hell is a talkie?
There's a blue syringe on the table, marked Lorazepam.

Should I help Galen or Rutherford?
That's up to you. Helping Galen is karmically neutral, though you gain karma from saving Terry. It involves getting the remote charge, disabling the IRC safeties, blowing up the holding tanks, collecting a key off Rutherford's corpse, freeing Merv and (if you saved Terry) the option of a new perk. If you help Rutherford, you lose karma for killing Galen, travel to the overthrown IRC in Dunwich Building, where you'll learn about the previous clinic and fight the specimens that overthrew the place. There's a new weapon to be found and when you report back to Rutherford, you'll get to choose between two different perks.

    ==== HELPING GALEN ====

Where do I get an extra suit for Terry?
If your medicine skill is high enough, you can save the agent who is unconscious in one of the clinic rooms and he'll give you his suit. Alternatively, if your repair skill is high enough, you can repair the two damaged suits in one of the research labs.

Do I have to save Terry?
You don't HAVE to, but it's the only way to gain a new perk from helping Galen.

Why do I die when I set off the bomb?
The initial wave of radiation will kill anyone not wearing a suit: make sure you're wearing yours.

    How do I free Merv?
You'll need the key off Rutherford's corpse.

    ==== HELPING RUTHERFORD ====

Do I really have to kill Galen?
    Yes.

    Where is the Geo-Terminal?
    In the staff quarters.

Is the S02 facility in the Dunwich Building?
Yes, but it is accessed from a nearly barricaded side door, not the main entrance.

Is it possible to get both perks from Rutherford?
No, you have to choose one or the other and they all carry a negative side in addition to their positive aspect. Personally, I really like the Molerat improved running speed.

If I don't like the perk, is there any way to change?
Not currently, but in the next update, I'll include a neutralizer for anyone who wants to get rid of the perk(s). For now, just save and try.

Is there any way to earn more than one reward?
Yes, it is technically possible to double-cross Rutherford, as long as you're careful about the order in which you do things. You can complete Rutherford's job, earning yourself a perk, then give Terry a spare radiation suit and blow up the holding tanks, earning you a perk from him.
