Some manually verified files
I got the image from the Elden Ring fandom wiki page.
The folder includes 4 .ico files. There is an optional .gif of the flasks but it sadly won't work as an icon. I'm just including the .gif because I already made it but there might be some 3rd party software to use gifs, idk.
Just download/extract, then right click game's shortcut >> Properties >> Web Document >> Change Icon ... >> then browse too and select the .ico file >> ok >> apply
