Just my own personal pet peeves corrected, but I figured I might as well see if anybody else is interested. Latest update changes/notes in bold.

IMMERSIVE MODEL SWAP

Swaps around certain models in an attempt to conform with canon appearances elsewhere, avoid overuse when possible, and also just general vibes.

- Metal Shields
Shield of the Resolute now uses the templar model (8003) to match the description of "a shield worthy of the knight commander."
Spirit of the 4:60 Black now shares the Dark Shield model (8007) to give it both a non-round appearance and the implication of the title.
The Shield of the Knight Herself now has Warstopper's model (8033) in an attempt to give it the actual mix of Orlesian and Dalish design implied by the lore.
Defender of the Wall takes the Reinforced Shield's model (8041) for vibes.
River's Wave takes Hearth Shield's model (8038) for vibes.
Wardwall now use Cornerstone's model (8001) to better match its designation as a metal shield and for vibes.
Cornerstone now uses the unused model 8050 - the game's files indicate it was meant for Aveline, but it looks...unfinished if that was the intention. Might as well use it to complete her companion quest, showing how far Jeven has fallen.

- Round Shields
Warstopper takes Wardwall's model (8002) to match its intimidating name and designation as a round shield.
Kirkwall Shield (model 8042) now bears the city's heraldry, as it should.
Shield of the Light/Flame/Faith uses Oddsmaker's model 8034 emblazoned with the Chantry's heraldry, as a nod to Origins' Templar Shield.
Ferelden Soldier's Shield now uses model 8036 (Blank Heraldic Shield) to match its round designation, with Ferelden heraldry.
Reinforced Shield now uses Oddsmaker's model (8034) to match the Inquisition shields of the same name.
Hearth Shield now uses Volcanic Shield's model (8037) for vibes and roundness.
Oddsmaker uses the afaik unused model 8030 (rough-looking kite shield) for the vibes.
Markham Head Stopper uses model 8043 for vibes.

- Longbows:
Recurve Bow now shares The Heavens' Answer's model (2135) to keep the name accurate while preserving the elvish implication of the Arlathan Longbow.
Cynoeswr Sain takes model 2131 from the Mariner Longbow to keep Bassath-Kata unique.
All three Mariner Longbows take the generic longbow (model 2103) because it is, unlike the original model, actually a longbow.
Borderfall now shares the Nevarran Lancer's model (2134) for vibe reasons.

- Shortbows:
Ferelden Shortbow now shares its appearance with the Weathered Bow (model 2023).
Undeclared Orlesian Longbow renamed to Undeclared Orlesian Shortbow, since that's what it is mechanically.
The Runt's Spiker now uses The Trepanner's Gift model (2020) for vibes and to avoid frequent duplication.
Crosscut Composite and Mercenary's Recurve now share model 2025 for vibes.
The Trepanner's Gift gets the Superior Shortbow model (2023) to avoid duplicates and to echo Origins' The Dark Moon.
Swarm and Plain/Superior/Enchanted Shortbow gets what was previously Mercenary's Recurve (model 2021)
because I like the implications of Chasind bow construction finding a niche in Kirkwall.

Truebow now uses the now-rare Cracked Bow model (2026) to continue to give Dalish weaponry some diversity back.

- Daggers:
The Arm of Adruil has been renamed to the Arm of Andruil, since it is pretty obvious that was the original intention.
The Blade of the Many now shares Finesse's model (440), taking inspiration from Finesse "[achieving] fame and adoration among the common folk."

Thrice-Bound now uses unused model 441 (appropriately vaguely crowbar-shaped).
Honeycut takes unused model 439 (double-bladed but curved in an abstract Dalish-y sort of way).
The Tiny Cut (413 of 1000) now shares its appearance with the Ragged Edge (model 436).
Coterie Shiv now uses unused model 443 (the evil crowbar, to keep the vibe of utilitarian thieves' guild).
Bassath-Katra takes the now-unused model 447 (previously the Coterie Shiv), thus remaining both Qun-adjacent and unique.
Bloodletter uses model 444--think of it as dried blood.
The Pairing Knife uses the previously-abundant jeweled model 405 because it looks nice.
Red Jenny Backbiter uses the previous Dalish model (438), so it can actually theoretically "bite back" and to continue the mystery of how far their organization reaches.

Tine and the Blade of Red Birth take the Dwarven Dagger model (431, used by the Noble Dagger in the Dwarf Noble Origin, among others).
All three Dwarven Dagger varients now use model 430 (Tine's old model, used by various Dwarven daggers in Origins including Sigrun's starting weapons).
Dalish daggers now use model 403 to match Origins' Dar'Misu.

- Longswords:
Royeaux One and Two now shares its appearance with Chevalier's Reach (model 150) to match its Orlesian origins as well as the Emperor's Guard Sword from Inquisition.
Overland Render takes the afaik unused model 114 (with the u-shape at the top).
Perrin's Nail takes the golden model that vibes with Kirkwall's heraldry (model 110, previously used by Royeaux One and Two).
Bassrath-Kata gets model 121, to match the Qunari Slasher found in Inquisition.
Raider Sword takes the afaik unused model 115 (sword as pick-axe), since it looks utilitarian and also matches pretty well Inquisition's Avvar Raider Sword.
The generic Dalish longswords now use model 104 to match Origins' Dar'Misaan, and have been renamed appropriately.
The Sword of the Antaam now shares model 118 with Sataareth to avoid Inquisition's Nevarran Noble sword.
Stens now use the Qunari Sten Sword model (117) to match Inquisition.

- Greatswords
"Imported" Antivan Blade now uses Potency's model (1002), because Antiva is flashy.
Binky's Comfort now looks suitably vaquely Qunari and vibes with its name (unused model 1007).
The Brothers' End sword that you get now resembles the one seen in the cutscene (model 1003, shared by The Celebrant among others).
Potency, aka Heartdrinker, also uses model 1003 to match up with Ageless from Origins and continue the implication of dwarven smith superiority.
The Blade of Mercy (unrestricted) now uses the same appearance as Fenris' version/the cutscene for the quest (model 1001).
Valo-Kas Eva now uses model 1031 to match its Inquisition appearance.
Valo-Kas Iss now uses model 1030 to match its Inquisition appearance.
Valo-Kas Katoh now shares Bassrath Kata's model (1050) to complete the visual evolution.
Red Grace now uses the unused model 1006 (seen as a Qunari weapon in Those Who Speak).

- Mauls
The Void's Hammer now shares The Oath-Breaker's model (1105) to avoid overuse and for vibes.
Caskethead's Portable Anvil now shares Sundering's model (1101) to connect the Dwarven craftmenship and keep Sata-Kas Iss unique to the Qun.
The Subtle Brute (model 1106) and Sata-Kas Iss/Eva/Katoh (model 1107) have had their models swapped to better match vibes and
Inquisition.

- Staves
The Acolyte's Staff (and its corrupted variant) gets its Origins appearance back (model 3035).
The Ornate Demonic Rod shares Stone's Breath's/Awakening's Darkspawn model (3031).
Enchanter's Staff gets its Origins appearance back (model  3034).
Free Man's Staff and the Apostate Staff also take model 3034 because lore-wise it is actually found in Lothering.
Staff of the Magister gets model 3052 (with the orb) to split the difference between Origins' similarly-named staves.
Eye of the Storm takes model 3005 to match Danarius's weapon, while Blemished gets the similar model 3004.
The Runed Staff also takes the Stone's Breath model (3031) for the vibes.
The Saartoh-Bas series of staves now use model 3013 instead of Freedom's Call's 3003 and have had their names corrected.
Both the fake prologue and the actual Champion Warstaffs now use model 3052 to match Iconic!Male!Hawke's trailer appearance.
The Tiger's Tale gets model 3008 because it looks more...tiger-y.
The Sylvan's Heart and the generic staves sold by Illen get model 3036 (Staff of the Adept/Morrigan's) to match the similarly-named Sylvan's Mercy from Origins.
The Crooked Staff now uses model 3033 so that it's actually a crooked staff.
The Hypnotist's Staff now shares model 3032 with Cold-Blooded for the vibes.
Valdasine also uses model 3032 to match the lore of it being "a metal staff ... that looks like lyrium."
The Staff of the Primal Order now shares Bethany's iconic/Act 3 model (3051) for vibes and damage type.
The Turncoat's Walking Stick now shares Freedom's Call's model (3003) just for vibes.
Chanters' Staff gets model 3011 to echo Mutiny (now model 3012) and avoid resemblance to Bassrath-Kata.
Aequitarian Fraternity Staff gets model 3008 too, just to remind everybody that this is Kirkwall and not Ferelden.
Wintersbreath has its Origins appearance back (model 3036).
Commissioned Enchanter's Staff and Mercenary's Staff also get model 3034 for the vibes.
Sketch's Resplit Shaft also gets model 3010 to more visually connect it with his Split Shaft from Leliana's Song while keeping the expanded designs of DA2.
Bassrath-Kata, to keep the theme of Qunari weaponry in DA2, gets model 3014 (resembles a hallibard and thus matches the Qunari ideals of usefulness).

- DLC Model Swaps
Hood's Message to the King (model 1302101) swaps with Righteous Rain (2131) by request.
Blade of Red Birth uses Jarvia's Shank's model (1300401) to match its Inquisition counterpart.
Cold-Blooded uses Apostate's Courage (model 603000) for vibes.

Just uti files separated into weapon type so you can pick and choose if you disagree with any choices.
