Improved Atmosphere includes a lot of item variation changes (the number that says to the game what a certain item should look like, essentially). So I think that the problem might be that you have some other mod that adds items to the game (like Kirkwall Exports) overriding Improved Atmosphere. This mod uses the same item variation slots as Improved Atmosphere used for its changes. But the models in this other mod don't exist for HM, so you end up with disembodied guards.
If you want to use the equipment changes in Improved Atmosphere, you probably should hunt down these item variation conflicts and discard the other mods that are causing them. Other solution might be deleting the .utc files in Improved Atmosphere (located mostly in 2.2. Characters, 5.2 Characters, 5.3. Companions, if I'm not mistaken) (and honestly, if you go this way, just throw away the whole 1.4 Items folder with them because it might just cause more problems like this somewhere down the line, though there are some .uti changes in some other folders as well, so you might also want to look out for them). If you decide to delete some things in Improved Atmosphere, though, it might be a good idea to take a look into its read me or whatever the manual is called because a lot of the folders are interdependent on each other, so you can proactively eliminate any potential future problems (if you delete 1.4 Items, delete any folder that depends on the 1.4 Items folder as well, for example).
