Alright, these are the forms:
Bear - Tank, aggro and melee
Blob - starter form, damage leech, damage absorption, can get you out if you're near death
Crab - Little, cute, lucky, use it for escape situations
Croc - Tanky, slow, more long-range options than the Bear
Frog - Water/Ice/Poison damage - not much armor
Owl - Swift aerial attacks, flies, evasive, dust-blinding
Mole - Plentitude of abilities that have to do with earth/oil and poison he can burrow into the ground too
Spider - Similar poison abilities with the Mole but more focused on entangled abilities instead of earth/oil
Tiger - Sneaky, close quarters, rogue-like character. (there's a cloaking ability just for that, the rest of the abilities no longer turn you invisible)
Turtle - He Heals and he can take a beating
Werewolf - You have it from the start, but needs source points to transfrom.. he can summon wolves, and has vampirism. He looks ugly AF though so I might make a version without it.
Wolf - Standard soldier like form. Good dps with some extra abilities.
I've removed every other form (Commander, Dragons, Tree, Forest, Unstable, Demon, Dragonling, Chaos, Djinn) since they're completely random and I don't think they go well with the druid we all know and love..
Following, are some comments from the previous author of the mod:

•MUST REMOVE WEAPON/SHIELD TO USE MELEE IN ANIMAL FORMS! PLAY THIS CLASS UNARMED!! (I'm not sure since I've tried with a twohanded sword and it works pretty well..)
•To change between forms, either wait for the timer to expire, or use the ability with the big red "X" to transform back to your original form.
I highly recommend that you keep your bar 1 clear (cycle through your skill bars by pressing 'R' and 'F'), and have only your shapeshifting skills there or on another bar. The abilities you get in animal form only appear on bar 1! Sorry!

- Fixed Blob's "god mode" (apparently he was invulnerable for 4 rounds)
- Made the prices of the books cheaper (or even completely free without shedding a sweat) ;)
- Adjusted some requirements for the Combat Abilities for each form
- More and more balancing
- Made a second version were the Werewolf and Blob are not anymore part of the druid mod (At start the skill presets will be empty.. simply choose a skill you like..)
1.6.6:
- Fixed Blob's lack of poison resistance, this little abomination is now immune to poison (since it's also his own blood),
- increased Mole's resistance.
- From this version on all forms won't be turned off on their own, you'll have to do it manually. If anyone wants to transform for a limited time, let me know and I'll make a version.

BUGS:
Unfortunately, this mod uses a lot of extra skills, so you might notice some duplicate icons when creating a new character, I know it's cluttery, but it'll be fixed after reviewing any further balancing issues you might post.

Some characters take a little damage from self-buff abilities (mend metal, fortify, etc).
