Download the main file. Extract the .7z file. Put the mods folder into your D2R main directory. Next, create a shortcut to D2R.exe in the main directory with "-mod AssassinRetouch -txt" command line in the shortcut's target line.
Move your save files(.d2s) from "C:\Users\[USERNAME]\Saved Games\Diablo II Resurrected" to "C:\Users\[USERNAME]\Saved Games\Diablo II Resurrected\mods\AssassinRetouch"
