Like in vanilla, some charms are now more rare than others, but there's a new option to equalize the rarities if one wants them all equally rare.

This D2RMM mod adds the Sundering Charms to the Hell boss item pools and optionally the regular unique item pool, as alternative ways to Terror Zones to find them.
This mod was originally named "Singleplayer Sundering Charms" as Terror Zones weren't implemented in offline games, rendering Sundering Charms unobtainable. But as of patch 2.6, Terror Zones now exist in offline. But this mod can still be used if one wishes to have alternative, more accessible, ways to find the charms.
Requires D2R patch 2.6 or later.
The charms will mainly drop from Hell bosses (primarily act end bosses, but also some mini-bosses like Blood Raven), and optionally also drop like any other unique in any zone, as long as that zone may drop ilvl 69 items (starting from early-mid Hell Act 1).
Includes config option to adjust the drop rate from bosses, optionally giving them equal rarity (originally, some charms are more rare than others) and an option to add the sundering charms to the regular (non-boss) item pool (note that this will make them compete with Gheed's Fortune; effectively lowering its drop rate).
