Just my take on what 'realistic' should mean, when it comes to combat.

Real Realistic Difficulty Mod
Simple and Dirty: The Realistic difficulty now comes much close to making the combat - well, realistic. 1 headshot (or 2-3 body shots) from a basic Pistol will kill almost any regular enemy. Illuminati Commandos might take 2 (4-5 to the body), Armored templars 3-4 (7-8 to the body). Bosses (Billie, Leo, Klara, JC, Paul, EndBoss Saman) 2-3 (5-6 to the body).
Shotgun is an insta-kill on almost any enemy, if you're up close. Armored Templars might take 2 blasts. Enhance it with EMP converter, they go down in 1 blast.
MagRail kills most enemies in 1 shot. Bosses and Commandos might take 2 (both have hardcoded resistance to energy damage). JC Denton will take 3 (he has more health then any other Boss character).
Sniper Rifle's scoped shot will 1-hit-kill any organic enemy in the game.
Bots are way easier to kill, obviously, but they will also end you in a second, if you let them.
Same applies to Alex. If you get caught out by a shotgun blast at close range, you'll die. No ifs or buts. You might survive 1 MagRail beam, if you're lucky. A few lucky pistol shots from a couple of thugs will end you easily. A turret will shred you to pieces in 2 seconds. Invest in aggressive defense drone. A frag/missile explosion anywhere near you WILL kill you. Invest in Hazard drone, too. Mag Bolt and Greasel hits will drain your health in 2-3 seconds without one. Basically - don't get hit, if you can help it.
Comparison to vanilla Realistic difficulty:
Damage NPCs takes from player: 3x (1.5x Vanilla).
Damage Player takes from NPCs: 4x (1.75x Vanilla).
Ammunition recovered in 1 ammo pickup: 25% of a full clip (75% in Vanilla).
So you'll dish out 2 times more damage, you will take 2.5 times more, and you'll only get 1/4 of a full ammo clip, on a pickup. Make the shots count. On the bright side, your damage-dealing augs are also way more deadly, given the damage multipliers.
Installation: Unzip the .ini file into the System directory within the game's main dir. Allow it to overwrite the existing one.
