Separate note: I was recently made aware that someone else already made a Rickroll screen hack (also without sound) but they named the mod in a way that makes it impossible to find for some reason. I'm not linking it here as I am not sure what their motive is for keeping it secret. I'm keeping my version available as a public service.
Downloaded this mod alongside the Diverse Death Screens mod, and apparently one of the diverse screens is the one for hacking a TV with Distract Enemies.
I discovered this during the quest to get Smasher's little friend at the Ebunike with Rogue. During the first gunfight , the Maelstrom heavy guy at the gate threw a Looney-Toons perfect grenade that landed at my feet and killed me, only for me to get f*#@ing Rick Rolled for my death screen. 10/10 experience, laughed so hard my stomach hurts.
That's alright, thanks. I figured out that it wants RED launcher to activate the mod every time I launch the game. That is slow and awful, and not for me. Sorry.
Check the forum, I'm making a How To for others to follow. I know it says "0" but there are posts there and this website is just bugged. I'll make a step for modifying audio later because it's a nightmare and I don't have time today to finish it, but if you just want to change the visual I finished making those steps:
Issue fixed in 2.2 also i don't use vortex, I manually manage all my mods. I had deleted r6/cache/modded to make sure it deployed but i assume there was an issue in 2.1.. Anyway Rick is showing up in 2.2, thanks!
I'm glad it's working. I just went in and lowered the volume a bit and set the attenuation to appear occluded at a shorter distance so the sound doesn't feel like it's following the player.

also, the mod does not deploy because you used problematic characters ("-" and ".") removed those, added a version number and it works
