Regarding the known issue of names not showing up until the default list has been exhausted, I believe that's because of the format you've used in the XML. If it were changed to follow the format of city names in the official files, it should allow the names to appear earlier.

If you mean <Row> versus <Insert>, no, it doesn't make any difference, they still do not show up until the main names have been used.
