Studious and anatomically minded, the Surgeon can administer potions to their allies and cripple foes with their knives. Based on homebrew by LaserLlama.

I've always been fond of non-magical doctor characters. Oh, sure, their job prospects are often harmed by Clerics and their damnable "Regenerate spells", but someone's got to dig up corpses and see how they tick.

This is another LaserLlama subclass. The Surgeon uses their skill in anatomy to maim foes with surgical precision: they have an expanded crit range and can replace their Sneak Attack damage with a collection of debuffs. The Surgeon can also use their Cunning Action to administer healing potions, by which I mean toss them at high-speed velocity. Fret not: potions thrown this way do not deal damage to allies, and I am assured this is a standard way to prescribe medicine.

There are a few changes from tabletop, as there usually are. Notably, your expanded crit range will stack with any other sources. You're also outright immune to diseases rather than just having advantage against them, so bully for you. Any features will automatically use the higher of Intelligence or Wisdom to calculate saves. Medicine is a Wisdom skill by default and it's probably the better stat, but it felt wrong not to allow for more academic surgeons.

Should be compatible with other Rogue subclasses with the Compatibility Framework. (Larian's progression merging is currently borked with non-level 1 multiclassing.) If you like any of my mods and want to show some extra support, I've set up a tip jar here:




Features by level

  • Level 3:
    • You are proficient in Medicine and Nature, and cannot roll lower than an 8 on a Medicine check. If your Intelligence is higher than your Wisdom, you may use Intelligence for your Medicine checks.
    • You may use your bonus action to throw healing potions.
    • Your crit range is expanded by 1 against humanoids, beasts, giants, monstrosities, and undead.


    • When you deal Sneak Attack damage, you may replace the extra damage with a debuff of your choice.
      • Cripple. You reduce a foe's movement speed; the amount reduced depends on your Surgeon modifier (equal to the higher of your Wisdom or Intelligence modifiers).
      • Daze. The target cannot take opportunity attacks against you this turn, and must subtract your Surgeon modifier from its next saving throw.
      • Infect. You force a Constitution saving throw; on failure, the creature is Poisoned until your next turn.
      • Maim. You force a Dexterity saving throw; on failure, the creature is Blinded until the end of your next turn.



  • Level 9: When you take Acid, Necrotic, or Poison damage, you reduce the damage by an amount equal to your proficiency bonus + your Surgeon modifier. Additionally, you have advantage against being Poisoned and are immune to diseases.
  • Level 13: Your expanded crit range increases to 2, and it now applies to all creatures other than constructs, oozes or plants. Moreover, when you apply a debuff with your Sneak Attack, you still deal Sneak Attack damage.
  • Level 17: You can use your Rogue level in place of a d20 for Medicine checks. Additionally, you can replicate the effects of Heal or Contagion once per short rest, though they must cast at melee range.
