Going prone will not break concentration. This is the only change.
There are 2 other mods that do a similar thing.
1. Prone Concentration
This downright breaks many interactions with Prone condition. You can see in its the comments section.
2. RAW Prone Paralyzed and Unconscious Patch
If you are looking for a more complete fix that is in line with 5e rules, I suggest going there. But, this mod introduced some very janky animations that I couldn't stand playing with so I made my own version.
I think I caught every reference to Prone, leave a comment if something is not working as expected.
