It all depends on the admin, if he turned it on, then both he and other players on his server will be shown a health bar. I even have a guide on how to enable it, if interested, then my comment with the link below (:
Seems to be a long running issue with the bar staying grey despite fighting zombies. Running version 17.4 Also tried running with anticheat off, no luck. Please look into this, thanks!
EAC means easy< anti cheat. Basically its an option when you host a server and it determines if the anticheat system is enabled or not. For this and many if not all other mods you need to disable it so you dont get banned

Go to:
\Steam\steamapps\common\7 Days To Die\Data\Config\XUi
Open windows.XML
Press Ctrl+f and look for <window name="windowTargetBar" controller="TargetBar" visibility="godmode">
Change <window name="windowTargetBar" controller="TargetBar" visibility="godmode">
to
<window name="windowTargetBar" controller="TargetBar" visibility="always">
or its just for ofline ?
