This mod doesn't touch on perk points at all, only on the amount of experience needed to level and the maximum level cap, so no, this mod would not cause you to get 5 perk points.  I will say that I've been playing on just vanilla settings in Alpha 21 and the first time I got any perk points I received 5 points, so it could just be vanilla being vanilla.

All of that being said, I've not specifically updated this mod for Alpha 21, yet.  Going off of what I've read in the docs, the main change that is needed is updating the ModInfo.xml for the new layout; the progression.xml should be functioning as intended still.

Dose this mod work server wide or does everyone have to install it?

I have not tested it on a server as I only play single player, but as far as I know mods that only make xml changes do not require client PCs to also install them, so it should work if on just the server.  If you test it and it does function that way, please let me know.

Hi! Tested in A20 - this mod is only needed by the server

Hello! Great work. I have a few questions. What are the XP settings set to? I mean 1000 levels I bet I will be running on 300% then. Also, you recommend custom perks to use this with because you have more points to spend. But custom perks is not updated in a while, does it still work in A20?

Ah, stupid me. I did not read the last line properly. It says you have not tested this with any perk adding mods for A20. I guess ill check it myself or look for a A20 updated perk mod. Thnx

For A20, the best recommendation I can give so far to have more perks is to use Lam's mod master which has been updated already for the new version.

What exactly do you mean by "XP settings" in this case?  I've made no changes to the options that can be altered in the starting world menu, if that's what you mean; this is a change to the progression.xml which halves the default base experience requirement.  In other words, if you start a new game and leave it at 100% XP, when you play you'll get the exact same amount of experience as you would in a non-modded game (i.e., kill a regular zombie and get +400 XP), except the impact will be different because you don't need as much experience in order to reach a new level.  Which is where it paradoxically making it harder comes into play, because you're comparatively leveling faster than you should be so you don't have as many resources as you would in a non-modded game.
