This is a shirt, that puts a glass in front of you. With this glass, you can also see clear in very murky waters.
To use it, equip it and you need to be in 1st person view and most likely need a weapon to be drawn. But you have to test this for yourself.
I don't know how it is with different camera mods.
This was not made to look pretty! It is just a helping object without enchantment.

You can find this in a barrel next to Anvils lighthouse (south of it).

It does not matter how murky the water is, it will always be completely clear through this glass.
