Well, pretty much what it says on the tin...

If I did not make any mistakes, this should help you get an esp file to make an already meshed replacement PA standalone instead. It does not contain information or help on mesh editing or on how to import meshes into FO4. Go here for the latter and to Google for the former.
There are probably better ways to do it, but this one worked for me and since several people requested this, here it is.
