A small modification for the mod Household Teleporter (http://www.nexusmods.com/fallout3/mods/7195), that mod download/pre-installation is a REQUIREMENT.
This simple/small modification to the original mod was made with FO3Edit.
DESCRIPTION
After the teleport happens, we get paralized for 10s, receive 100 radiation damage, 50 hp damage, 10 cripple damage to all limbs, 25 cripple to head, 50 cripple to torso, to make it more hardcore!
HELP WANTED
I was unable to make that happen when reaching home tho, only when teleporting back to the wasteland 
I was unable to add visual effects unfortunately, I thought on smoke burning effects and also the robot explosion electric one, would be great!
Add an option to spend a fission battery, to cope with FWE breeder, and would be an interesting energy spending mode 
Add randomicity, making all damage and radiation be minimum half and maximum double of default values.
Make all the damage and radiation happen faster or instantly...
