The Triple Perk Mod allows you to purchase up to three levels of the perks: Action Boy/Girl, LifeGiver, StrongBack, & Toughness.
It was manufactured with the G.E.C.K.
It is a .esp file, install it by placing it in the Data folder in the FallOut 3 directory. Then activate it from the launcher with all your other data files.
