This mod was created by Fallout 3 Nexus user: amusement
I did not create this mod, I am simply uploading it to the Nexus site so others can enjoy it. With that said, I apologize in advance for any mistakes that may be in this ReadMe :P
Details:
This is a mod that allows 2 players to play Fallout 3 cooperatively on the same screen. In order for this to be done, one player will have the normal gameplay view, while the second player is controlling a seperate in-game character. Due to limitations of the FO3 game,
so far, split screen capabilities are not available.
(If anyone has ideas for implementing this, please help!)
So the second player relies on the first player's view.
In other words, player 1 will have the option to be in first or third person, but Player2 will always be in third person view and need to be within the view of Player1 in order to see where he's going. Also, Player2 is set to essential (cannot die) and has an assortment of autofire weapons with infinite ammo. (Nice tradeoff, eh?)
It can be a little tricky to get used to at first, but it REALLY works! You and someone else can actually exist inside the same game of Fallout 3! Finally, you can have a follower smarter than any NPC! Well, hopefully you have a friend that is smarter than an NPC ;)
Instructions:
Place the .esp file into your Fallout 3 data folder
(usually C:\Program Files\Bethesda Softworks\Fallout 3\Data)
Start Fallout Mod Manager and check the mod. (I have not tested this without FOMM, but can't see why this wouldn't work without it)
Play!
The second player character should be standing by the gate inside Megaton wearing a radiation suit. Once you see him, press the keys that are assigned to control him, and another player should now be able to play as him.
The keys are mapped as follows:
I: Move forward
K: Move backward
J: turn left
L: Turn right
M: Look up
N: Look down
O: Change weapon
P: Attack
U: Jump
H: Summon Player2 to Player1's location
Uninstallation: I unchecked the mod from FOMM and the other character vanished. Easy!
Compatibility: I can see no conflicts with any other mods, as all this mod does is add a new controllable character to a location in Megaton. Only possible conflict that I can see would be anything that messes with his original spawn point, and any commands that are assigned to the mapped keys above might interfere.

Please post your comments/feedback on the Nexus page, especially if you like it and want additions/improvements.
Let them know so they can continue work on it! I am interested to know if this handles well with a gamepad, so if you got one and would like to try it out using a program like Joy2Key, go for it! Or you could try using a second keyboard =)
