-Download ReShade 3.0.5 here and Put "ReShade_Setup_3.0.5" in ...Dark souls III\Games
-Run the Reshade setup and choose DarkSoulsIII.exe, then select direct3D 10 and download all the effects.
Download the mod and put the file "DARK OMEN RESHADE.ini" in ...Dark souls III\Games
-Run the game and ReShade automatically load the shaders and apply them
If not, open ReShade in-game with "F11" and choose "DARK OMEN RESHADE.ini" right at the top of the ReShade window.

TIPS
YOU CAN TOGGLE ON/OFF SOMES EFFECTS IN GAMES with these HOTKEYS (Can Improve system performance)
BORDER: num 3 (enable by default)
DOF (matsoDoF): num 5(disable by default) enable it discrease performance
tiltshift: num + (disable by default) enable it slighty discrease performance
Ambient light: num 7(enable by default) disable it increase performance
LiftgammaGain: num - (enable by default) disable it change the atmosphere of the game, more brighter and slighty increase performance

the game is stable and relatively Fluid.
For experienced users of the reshade program:
i used this list of fx:
tomemap, levels, vignette, denoise, border
hdr, ambiant light, liftgammagain, adaptive sharpen, highpassshap, curve, DOF (matso)
You can set these fx to change the atmosphere of the game and put it to your taste via Reshade windows (F11 hotkey)
