Custom HUD I did a while ago because I wanted something different for the humanity counter. Features a reskinned HUD, brighter enemy healthbars, new aiming reticle and new boss HP bar.

The mod used for the beautiful status icons is Revamped Stat Buffs by SilentRequiem http://darksouls.nexusmods.com/mods/8/ - they are not part of this mod but appear in the screenshots.
